Available to March 15th.  From the site "With your help, we are building a legacy of health information for future generations and arming researchers with valuable data that may shed light on the prevention and treatment of diseases like cancer, diabetes, heart disease, asthma, Alzheimer’s and depression." You do not have to enter your Health card number! You can leave it blank and still get the gift card. There are lots of personal questions but you can hit "prefer not to answer". At the end you will be able to pick your gift card from different stores.
